The movie introduces Dr. Jack Gramm (Pacino), a university professor and forensic psychiatrist for the Federal Bureau of Investigation, who with his staff and students are celebrating the conviction of a serial killer, the Seattle Slayer. He is shown dancing and drinking, the next morning he wakes up and finds himself with Sara Pollard, the girl he came home with the previous night. He then dresses for work while Sara makes him breakfast, and receives a call from his secretary Shelly - Frank Parks an FBI investigator (William Forsythe), tells him that there has been a murder, in the style of the Seattle Slayer.
Dr. Gramm leaves for office in a taxi where he meets with his secretary Shelly (Amy Brenneman), who tells him that the FBI is in his office and his teaching assistant Kim Cummings(Alicia Witt) is waiting for him. The FBI suspects that there is a copycat murderer or someone is trying to prove that Jon Forster is innocent. Gramm believes the second theory, however the FBI question him about his relationship with Dale Morris, one of his students, the one who was killed the previous night. He is shown a tape in which Dale reveals 'Dr. Gramm got the wrong man', just before she was brutally murdered.
Gramm goes to the university and on the way to his class he gets a phone call saying that he has 88 minutes to live, by an anonymous caller. He gets Shelley to trace the number and find about the people who have visited Forster since he thinks Forster is behind everything, back in jail. In his class he receives two more phone calls threatening him. At the moment Jack Gramm is first told he has only 88 minutes to live, the remaining running time of the motion picture, including final credits, is 88 minutes.{{Fact|date=August 2007}}
The guy in leathers who was outside his house in the beginning of the movie comes into Gramm's class and stands at the back. After receiving the second threatening phone call, Gramm looks around the class and sees a student doing something in his phone, he goes over and questions him but the boy was just checking up on some scores. The guy in the leathers then leaves, Gramm follows him, but he has disappeared, he then goes back to class but the dean soon comes in and tells everyone to evacuate the building because there was a bomb threat.
Gramm then goes to the car park to get his car but his car has been vandalised, his gun stolen and on the windscreen it was lettered that he has 72 minutes remaining. He then sees Kim, borrows her cell-phone since he broke his own, & tells her to meet him outside the garage in ten minutes with his car. He hears cries for help, and finds that Lauren, one of his students, had been attacked. He takes her to the campus security office and files a report on his car as well. On the way home, he finds out that the guy in leathers who Kim said is her ex-boyfriend is actually her ex-husband who is afraid that she is going to be taken advantage of by Gramm.
On arriving at his apartment he is told by the security that there was someone in leather clothes and motorbike helmet waiting for him and there is a package for Gramm. Kim says that she thinks it was Guy Laforge her ex. On reaching the apartment he opens the package to find a tape recorder which plays the assault of Gramm's sister Katie. Since there are just 2 copies of the tape, one with the NYPD and the other locked in his office, he thinks someone close to him might be behind this. He then calls MSNBC to talk to Forester live. Guy then appears and the apartment is set on fire so that they have to leave but Guy is shot by someone wearing black leather and a black motorbike helmet.
On escaping the building, they try to open the car but it blows up. They hire a cab and on the way Jack explains to Kim about the tape, his sister and the story behind the '88 minutes'. They reach Sara Pollard's house who happens to be an escort girl, hired by someone else to sleep with Gramm so that he could be framed in all of the murders. But she is found murdered in exactly the same manner as Dale. Shelley arrives and they leave but Kim has disappeared.
Outside, Frank Parks holds Gramm at gunpoint and says that he is a suspect in the death of Dale Morris, but he convinces the detective to give him some more time. He then receives a phone call from Carol who tells him to meet him at his office in the university. While in the taxi he is looking at some of the people who have been meeting Jon Forster for the past couple of weeks and sees a woman's name which triggers a negative reaction.
He leaves a message for Kim to not go to the university on her Blackberry, since he is still using her phone. But then she calls him, telling him to meet her at his office. On arriving there, he meets Mike Stamp who confronts him and says that he found Lauren's bag, a bottle of a drug used to knock out the girls by the murderer and a knife.
He then gets a call to meet Kim at the stern building on the seventh floor, but Mike is blocking the door and won't let him leave so he shoots the door close to Mike's ear, making him move away and runs off to the Stern building and calls Frank to meet him there. While there he finds the doors to the elevators closes, then he hears dripping and blood drips onto his coat, upon looking up he sees Carol hanging, he runs to the stairs and goes to the seventh floor but finds that his gun is empty. He then meets the killer and sees that it is Lauren Douglas, his student, who has been doing all the killing and threatening. She reveals that Forster manipulated her into forcing Gramm to confess to coaching Janie Kay and falsifying evidence, so that Forster could be released. Upon Gramm's confession, she calls Forster and tells him to call her back. She is then shot by Parks from across the floor and Carol is saved by Gramm, and Lauren falls down to the floor. Forster calls Lauren from the prison and Gramm gives him the bad news that Lauren is dead, which makes his smile fade to fear since she was his last hope of survival. Lauren was his lawyer and that had given him unlimited access to her, so he could plan the murders right at the prison.
The last scene has Gramm and Kim walking with her asking if he did actually falsify the evidence, he tells her to look at the difference between Justice and Truth and asks her what she would have done and says don't answer. Then he goes to his class where he talks to the relatives of some of the slayers' victims and his students about torture felt by the loss of loved ones. Then, in his usual teaching style he asks the audience what one does before entering a court, and thus the credits roll.
